What to Bring to Your Initial Appointment

doctor appointmentWhen you make an appointment with a physician to discuss treating a pre-existing condition or pain, there are likely a lot of things running through your mind. Determining what exactly to bring to your initial appointment is difficult. That’s why we have compiled this list of items to bring with you to your appointment so everything runs as smoothly as possible.

Yourself (and your ID)

First and foremost, get yourself to the appointment. And don’t forget to bring with you some form or identification (valid driver’s license, passport, etc.). This is for your own security. We need to make sure you are who you say you are. It’s not that we don’t trust you, we do our utmost to protect your identity.

Medical Records

Also remember to bring any medical records you might have. This can include results from previous imaging tests (x-rays, CT scans, or MRI scans), as well as reports of any past surgeries, physical therapy, chiropractic treatment, EMGs, etc. If you previously had an MRI, CT, or x-ray scan done through Dr. Sinicropi, we will have that information on hand. If not, please contact the hospital or clinic that took the tests and bring them with you to your initial appointment for the doctor to review.

Insurance Information

Next, bring your insurance card and provider information with you, as well as referral or pre-authorization if your insurance requires it. If your plan requires it, also remember to bring any necessary co-payments as well. If your treatment involves a personal injury claim, please bring your claim information and file number so we can appropriately document that.

Hopefully this guide will help you come prepared to your initial appointment. It may seem like a lot to keep track of, but once you get all the paperwork and documentation out of the way, you can focus on your treatment and recovery.
